Your ID card provides policy information, and, for Medicare supplement insurance plan claims, informs your provider that no claims need to be filed with Provident American Life and Health Insurance Company (PALHIC).

If you have lost your ID card, call one of our Customer Service Representatives at 1-866-459-4272 for a replacement.  You can also order a replacement ID card online by filling out the following ID Card Replacement Request form
